1. Parturition (Childbirth) – Definition
Parturition, commonly known as childbirth, is the natural process by which a fully developed fetus is expelled from the uterus at the end of pregnancy, usually between 38 and 40 weeks. It marks the completion of pregnancy and the beginning of independent life for the newborn. Parturition involves coordinated uterine contractions, cervical dilation, and hormonal changes that allow the baby to pass through the birth canal. After the baby is delivered, the placenta is also expelled. The process is carefully regulated by hormones such as oxytocin, prostaglandins, and relaxin, ensuring safe delivery of both the baby and the placenta.
2. Stages of Parturition
Parturition occurs in three distinct stages. The first stage, called the dilation stage, begins with regular uterine contractions, causing the cervix to soften, thin, and dilate from about 1 cm to 10 cm. The second stage, known as the expulsion stage, involves powerful contractions that push the baby through the birth canal until delivery. The third stage, called the placental stage, occurs after the baby's birth when the placenta and fetal membranes separate from the uterine wall and are expelled. Each stage is essential for ensuring a safe and successful childbirth process.
3. Hormones Involved in Parturition
Several hormones work together to regulate childbirth. Oxytocin, released from the posterior pituitary gland, stimulates strong rhythmic uterine contractions through a positive feedback mechanism. Prostaglandins, produced by the uterus, placenta, and fetal membranes, strengthen uterine contractions and help soften the cervix for delivery. Relaxin, secreted by the corpus luteum and placenta, relaxes pelvic ligaments and softens the cervix, allowing easier passage of the baby through the birth canal. The coordinated action of these hormones ensures effective labor, smooth cervical dilation, and successful delivery while minimizing complications during childbirth.
4. Initiation of Parturition
Parturition begins when the fully developed fetus exerts pressure on the cervix at the end of pregnancy. Stretch receptors in the cervix are stimulated and send nerve impulses to the hypothalamus. The hypothalamus signals the posterior pituitary gland to release oxytocin into the bloodstream. Oxytocin stimulates stronger uterine contractions, which increase cervical stretching. This additional stretching triggers the release of even more oxytocin, creating a positive feedback mechanism. The cycle continues with increasingly powerful contractions until the baby is delivered. This natural feedback process is essential for initiating and maintaining effective labor during childbirth.
5. Flow of Events During Parturition
The sequence of childbirth follows a well-organized series of events. First, the fetus reaches full maturity at 38–40 weeks of pregnancy. The fetal head stretches the cervix, triggering the fetal ejection reflex. This stimulates the release of oxytocin and prostaglandins, which produce strong and rhythmic uterine contractions. As contractions become stronger, the cervix dilates completely, allowing the baby to move through the birth canal. After the baby is delivered, mild contractions continue, helping the placenta separate from the uterine wall and be expelled. This coordinated sequence ensures safe delivery of both the baby and placenta.
6. Causes That Help Initiate Parturition
Several physiological changes contribute to the onset of labor. An increased estrogen-to-progesterone ratio enhances uterine sensitivity to contractions. Higher levels of prostaglandins promote cervical softening and stronger uterine contractions. The myometrium becomes more responsive to oxytocin as pregnancy reaches full term. Mechanical stretching of the uterus and cervix by the growing fetus activates stretch receptors, initiating the fetal ejection reflex. Additionally, increased fetal cortisol production near term helps mature fetal organs and stimulates hormonal changes that promote labor. Together, these factors prepare the uterus and cervix for the safe and effective process of childbirth.
