SHOCK
1. What is Shock?
Shock is a life-threatening medical emergency in which the body's tissues and organs do not receive enough blood and oxygen. This inadequate blood circulation leads to reduced oxygen delivery, causing cells to malfunction and organs to fail if not treated quickly. Shock can result from severe blood loss, heart failure, infection, allergic reactions, or nervous system injury. Without immediate medical intervention, shock may progress to irreversible organ damage and death. Early recognition of symptoms such as low blood pressure, rapid pulse, pale skin, and confusion is essential. Prompt treatment helps restore blood circulation and improves the patient's chances of survival.
2. Hypovolemic Shock
Hypovolemic shock occurs when the body loses a large amount of blood or body fluids, reducing blood volume and preventing adequate circulation to vital organs. It is the most common type of shock. Common causes include severe bleeding (hemorrhage), dehydration, burns, vomiting, and diarrhea. Symptoms include rapid heartbeat, low blood pressure, cold clammy skin, weakness, dizziness, and reduced urine output. Treatment focuses on stopping blood loss, replacing fluids with intravenous solutions or blood transfusions, and maintaining oxygen supply. Early diagnosis and rapid fluid replacement are essential to prevent organ failure and improve survival.
3. Cardiogenic Shock
Cardiogenic shock develops when the heart cannot pump enough blood to meet the body's needs. The most common cause is a severe myocardial infarction (heart attack), although heart failure, abnormal heart rhythms, or damaged heart valves can also cause it. Poor heart pumping leads to reduced blood flow to organs, causing low blood pressure and inadequate oxygen delivery. Symptoms include chest pain, shortness of breath, cold sweaty skin, rapid pulse, and confusion. Treatment includes oxygen therapy, medications that improve heart function, emergency angioplasty, or surgery. Immediate medical care is essential because cardiogenic shock has a high mortality rate.
4. Septic Shock
Septic shock is a severe complication of infection in which bacteria, viruses, or fungi trigger an overwhelming immune response. This response causes widespread inflammation, blood vessel dilation, and dangerously low blood pressure, reducing blood flow to vital organs. Common sources include pneumonia, urinary tract infections, abdominal infections, and bloodstream infections. Symptoms include fever or low body temperature, rapid heartbeat, fast breathing, confusion, and extremely low blood pressure. Treatment involves immediate administration of intravenous fluids, broad-spectrum antibiotics, oxygen therapy, and medications called vasopressors to maintain blood pressure. Early treatment greatly improves survival and prevents organ failure.
5. Anaphylactic Shock
Anaphylactic shock is a severe, life-threatening allergic reaction that occurs suddenly after exposure to an allergen. Common triggers include bee stings, certain foods (such as peanuts or shellfish), medications, and insect bites. The allergic reaction causes widespread release of histamine, leading to airway swelling, breathing difficulty, low blood pressure, and circulatory collapse. Symptoms include hives, facial swelling, wheezing, dizziness, rapid pulse, and loss of consciousness. Immediate treatment with intramuscular epinephrine (adrenaline) is lifesaving. Additional treatment includes oxygen, intravenous fluids, antihistamines, and corticosteroids. Patients should receive emergency medical care immediately after the reaction.
6. Neurogenic Shock
Neurogenic shock occurs when damage to the spinal cord disrupts the normal control of blood vessel tone by the nervous system. As a result, blood vessels widen excessively, causing severe low blood pressure and reduced blood flow to vital organs. Unlike other forms of shock, patients may have slow heart rates (bradycardia) instead of rapid pulses. Common causes include spinal cord injury, severe head trauma, and certain neurological disorders. Symptoms include warm, dry skin, dizziness, weakness, and hypotension. Treatment includes intravenous fluids, medications to increase blood pressure, spinal stabilization, and management of the underlying neurological injury.
7. Common Symptoms of Shock
Although symptoms vary depending on the type of shock, several features are common. Patients usually develop cold, pale, clammy skin, a rapid, weak pulse, low blood pressure, and fast breathing as the body attempts to maintain blood flow. Reduced blood supply to the brain may cause dizziness, confusion, anxiety, restlessness, or fainting. In severe cases, the patient may become unconscious. Reduced urine output is also common because the kidneys receive less blood. Recognizing these warning signs early and seeking emergency medical help immediately can prevent complications such as organ failure, permanent damage, and death.
8. First Aid for Shock
First aid for shock should begin immediately while waiting for emergency medical services. Call for medical help without delay. Lay the person flat on their back and, if there is no head, neck, spinal, or leg injury, raise the legs about 20–30 cm to improve blood flow to vital organs. Control any external bleeding and keep the person warm using a blanket. Do not give food or water if the person is unconscious or vomiting. Monitor breathing and pulse continuously. If breathing or heartbeat stops, begin Cardiopulmonary Resuscitation (CPR) until professional medical assistance arrives.
