1. Heart Valve Disorders (Definition)
Heart valve disorders occur when one or more of the heart's four valves fail to open or close properly, disrupting the normal flow of blood through the heart. The four valves—mitral, aortic, tricuspid, and pulmonary—ensure that blood moves in one direction. When these valves become narrowed, leaky, or weakened, the heart must work harder to pump blood efficiently. Heart valve disorders may be present at birth or develop later due to aging, infections, or other heart diseases. If left untreated, severe valve disease can lead to heart failure, arrhythmias, stroke, and reduced quality of life.
2. Four Heart Valves
The human heart contains four valves that regulate one-way blood flow. The mitral valve lies between the left atrium and left ventricle, while the aortic valve is located between the left ventricle and the aorta. The tricuspid valve separates the right atrium from the right ventricle, and the pulmonary valve lies between the right ventricle and the pulmonary artery. These valves open during blood flow and close afterward to prevent backward movement of blood. Healthy heart valves ensure efficient circulation and proper oxygen delivery to the body. Damage to any valve can significantly affect heart function.
3. Stenosis
Valve stenosis is a condition in which a heart valve becomes narrowed or stiff, restricting blood flow through the valve. The heart must generate greater pressure to pump blood across the narrowed opening, increasing the workload on the affected chamber. Common causes include aging-related calcium deposits, congenital valve abnormalities, and rheumatic fever. Symptoms may include chest pain, shortness of breath, dizziness, fainting, and fatigue. Severe stenosis can eventually lead to heart failure if untreated. Diagnosis is mainly made by echocardiography, and treatment ranges from medications to balloon valvuloplasty or surgical valve replacement, depending on severity.
4. Regurgitation (Valve Insufficiency)
Valve regurgitation, also called valve insufficiency, occurs when a heart valve does not close completely, allowing blood to leak backward instead of moving forward. This backward flow forces the heart to pump extra blood, increasing its workload and eventually weakening the heart muscle. Regurgitation may result from valve damage, infection, rheumatic fever, congenital defects, or degeneration with age. Symptoms include fatigue, shortness of breath, palpitations, and swelling of the legs. Mild regurgitation may require only regular monitoring, while severe cases often need medications, valve repair, or valve replacement surgery to restore normal circulation.
5. Valve Prolapse
Valve prolapse occurs when the valve leaflets bulge backward into the chamber instead of closing normally during heart contraction. The most common form is mitral valve prolapse (MVP), where the mitral valve bulges into the left atrium. Many individuals have no symptoms, while others experience palpitations, chest discomfort, dizziness, fatigue, or mild shortness of breath. In some cases, prolapse causes mitral valve regurgitation, leading to blood leakage. Diagnosis is confirmed by echocardiography. Most cases require only observation, but severe prolapse with significant leakage may need medications or surgical repair to prevent complications.
6. Causes of Heart Valve Disorders
Heart valve disorders can develop from several conditions. Congenital heart defects cause abnormal valve formation present at birth. Rheumatic fever, resulting from untreated streptococcal infection, can permanently damage heart valves. Aging causes calcium deposits that stiffen valves, particularly the aortic valve. Infective endocarditis, a bacterial infection of the heart lining, can destroy valve tissue. A heart attack may damage muscles supporting the valves, causing valve dysfunction. Other causes include connective tissue disorders, high blood pressure, radiation therapy, and degenerative changes. Identifying the underlying cause helps guide appropriate treatment and prevent disease progression.
7. Symptoms of Heart Valve Disorders
Symptoms depend on the affected valve and severity of disease. Common symptoms include chest pain, shortness of breath, especially during exercise, and persistent fatigue due to reduced blood flow. Patients may experience dizziness, fainting, irregular heartbeat, or heart murmurs, which are abnormal sounds heard during heartbeat. Fluid accumulation may cause swelling of the legs and ankles. Some individuals remain symptom-free until the disease becomes advanced. Severe valve disorders may lead to heart failure if untreated. Early recognition of symptoms and timely medical evaluation improve treatment outcomes and reduce long-term complications.
8. Diagnosis of Heart Valve Disorders
Diagnosis begins with a medical history and stethoscope examination, during which a doctor may detect a heart murmur. The gold standard investigation is echocardiography (Echo), which uses ultrasound to visualize valve structure, movement, and blood flow. Additional tests include electrocardiography (ECG) to assess heart rhythm, chest X-ray to evaluate heart size and lung congestion, and cardiac CT or MRI for detailed imaging. In selected patients, cardiac catheterization may also be performed. These investigations determine the type and severity of valve disease and help doctors plan the most appropriate treatment.
9. Treatment of Heart Valve Disorders
Treatment depends on the type and severity of valve disease. Mild cases may require only regular monitoring and lifestyle modifications. Medications such as diuretics, beta-blockers, anticoagulants, and blood pressure-lowering drugs help relieve symptoms and reduce complications. When valve damage becomes severe, valve repair surgery is preferred whenever possible because it preserves the natural valve. If repair is not feasible, valve replacement surgery using mechanical or biological valves is performed. Some patients may benefit from minimally invasive procedures such as transcatheter valve replacement. Regular follow-up is essential to monitor heart function and treatment effectiveness.
