Chandipura Virus: A Rapidly Progressing Viral Encephalitis
Context: Chandipura virus (CHPV) has again drawn attention during the monsoon season because of suspected and confirmed cases of acute encephalitis, particularly among children in Gujarat. The disease is important for public health because neurological deterioration can be very rapid.
Salient Features: CHPV is a negative-sense, single-stranded RNA virus belonging to the Vesiculovirus genus of the Rhabdoviridae family. It is mainly associated with phlebotomine sandflies and primarily affects children. Typical symptoms include sudden high fever, headache, vomiting, seizures, altered consciousness and, in severe cases, coma. ICMR-NIV describes it as an important cause of acute encephalitis syndrome in children with historically high fatality in severe outbreaks.
History: The virus was first isolated in 1965 in Maharashtra. Major outbreaks occurred in central and southern India during 2003–04. In 2024, India experienced its largest Chandipura-associated AES outbreak in about two decades; WHO reported 245 AES cases and 82 deaths up to 15 August 2024, including 64 laboratory-confirmed CHPV infections.
India’s Response: India follows surveillance, laboratory diagnosis and vector-control measures through the Health Ministry, NCDC, ICMR and state health authorities. ICMR-NIV has also undertaken outbreak investigations and vaccine research.
Current Status: There is presently no licensed vaccine or specific antiviral treatment. Management is mainly supportive. Early recognition, rapid hospitalisation, seizure management and sandfly control therefore remain crucial. The disease underlines the need to strengthen AES surveillance, rural healthcare and vector control, especially during the monsoon.

1. Why should Chandipura virus be treated as a serious public-health concern even if the number of cases is relatively limited?
Answer: The concern is not only the number of cases but the speed and severity of illness. Children can deteriorate quickly after fever and neurological symptoms begin. A local outbreak can therefore overwhelm rural hospitals. Early detection, referral systems and intensive-care readiness become as important as disease surveillance.
2. Since there is no specific antiviral drug or vaccine, what should be the main focus of government policy?
Answer: The priority should be prevention and early treatment. Government should strengthen sandfly control, village-level surveillance and awareness among parents and health workers. Suspected children should be referred quickly. Hospitals in vulnerable districts should have clear protocols for seizures, fluids, ventilation and other supportive care.
3. Why are rural children particularly vulnerable to diseases such as Chandipura virus?
Answer: Rural exposure to vectors may be higher because of housing conditions, animal shelters and environmental factors. Access to specialised healthcare can also be slower. A child with fever and seizures may lose crucial treatment time during referral. So better primary healthcare and transport links can directly reduce deaths.
4. What lesson does the Chandipura outbreak offer for India’s disease-surveillance system?
Answer: India needs surveillance that can detect unusual clusters before they become large outbreaks. Laboratory networks should work closely with district hospitals. Data from fever and encephalitis cases should be analysed quickly. Local health workers are also important because they often notice the first change in disease patterns.
5. Can vector control alone prevent future Chandipura outbreaks?
Answer: No. Vector control is necessary, but it is only one part of the response. India also needs better housing, sanitation, community awareness, rapid diagnosis, trained medical staff and strong referral systems. Research on vaccines and treatment should continue. A combined public-health approach is likely to be more effective.
